Putative Father Details

On this screen you record the details for a putative father who acknowledges that he has fathered the child and will claim responsibility for that child. You enter the required information from individuals who intend to claim paternity of a child that is born out of wedlock.

Click Link Father to select the father who is claiming paternity and responsibility for the adoptive child.

Fields on this screen

Child's Information	Enter the name, gender, birth date, and place of birth details for the child.
Mother's Information	Enter the name details for the child's mother.
Father's Name	Displays the name of the person you selected in the search.
Father's Address	Displays the address of the selected father.
Date Address Changed/Registered	Type of select the date when this address was changed or registered.
Father's Telephone Number	Displays the telephone number of the father.
Father's Social Security Number	Displays the Social Security Number of the father.
Ineligible Record	Select this check box to indicate the record is no eligible
Date Form Signed	Type or select the date when the father signed to claim responsibility.
Date Revoked	Type or select the date when the form was revoked, if applicable.
Appeal Date	Type or select the date when the father appealed, if applicable.
Comments	Enter additional information about the selected father.
New Comments	Enter additional information about the selected father.
