Temporary Break in Placement Details

Enter the following:

• Temporary Break Date*  - enter the date or use the calendar icon to record the start date of the break

• Temporary Break Type* - choose one of the three choices for a Temporary Break

• Detention

• Hospital Admission

• Runaway

• Return Date - once the youth has returned from the Temporary Break, enter the date or use the calendar icon to document the return.

• Estimated Return Date - optional information if known

Resource Information

If the Temporary Break is for a Detention or Hospital Admission the name of the placement needs to be recorded to insure whereabouts of children at all times.

If the Temporary Break is for a Runaway instance there is no Resource to be linked.

• Resource Type -  Residential Facility

• Resource Sub Type -  Detention

• Organization – DCS Central Office

• Search

Resource Search Tab – for Hospital Admission

• Resource Type – Non-DCS

• Resource Sub Type – Hospital

• Organization –  DCS Central Office

• Search

• Click Save
