On this screen you to view the summary information about a request for State-funded Subsidized Permanent Guardianship (SPG). This screen displays whether the child and resource are eligible and details about the eligibility.

Application Received Date	Enter the date the Department received the family's application for SPG.
Projected End Date	Enter the estimated date when the subsidy should end.
Approval/Denial Date	Enter the date when the application was approved or denied.
Child Part of a Sibling Group Exception	Indicate whether the child is part of a sibling group.
Comments	Enter additional information about the request for the subsidy.
Subsidy Rate Type	Select the type of subsidy rate, such as per diem or monthly.
Recommended Per Diem Amount	Enter the amount of per diem recommended for the subsidy.
Approved Per Diem Amount	Enter the amount of per diem that was approved for the subsidy.
