Service Description

Select the service that is applicable to the cost you are defining.

TennCare Approved

Select this check box to indicate that TennCare approves the cost for the contract.

Rate

Enter the number of units of the service included in the contract.

Cost Begin

Enter the date when the resource can begin assessing this cost. This date cannot be earlier than the contract date.

Cost End

Enter the date when the resource may no longer assess this service cost. This date cannot be later than the contract ending date.

Specific Person

If this cost is applicable to a specific child in the care of the resource, click Link Person to select the child.

Add On Cost

Enter the additional contracted cost for the selected child. Click Calculate to display the new total for the service cost.
