Adjudication Notification to Schools CS-0703

CS-0703 is generated to notify school Principal if a youth is adjudicated for certain delinquent Adjudications.

The Purpose of this form is to provide written notification to the Principal about the adjudication of Child/Youth. Form provides information about the Child/Youth have appeared before Court of County, Tennessee regarding any charges applied to them.

User Entry Parameters

School Principal's Name	Enter the Principal's Name. Mandatory.
Name of School	Enter the name of the school for which letter is generated. Mandatory.
School City	Enter the school city. Mandatory.
School State	Enter school state. Mandatory.
School Zip	Enter the school zip code. Mandatory.
Region/Office/Facility	Enter the FSW's region or office or facility. Mandatory.
Address	Enter the address of the Family Social Worker. Mandatory.
FSW Family Services Worker City	Enter Family Social Worker City Information. Mandatory.
FSW Family Services Worker State	Enter the Family Social Worker State. Mandatory.
FSW Family Services Worker Zip	Enter the Family Social Worker Zip Code. Mandatory.
Telephone Number	Enter Telephone Number of Family Social Worker.
Court	Enter court name. Mandatory.
County	Enter the county. Mandatory.
