CS-0816 is generated for presenting to the court at or immediately prior to the Adoption Finalization hearing.
CS-0816 includes child information and their family member information from person profile. In addition, it includes information regarding the adoptive family from the resource home record. This form requires signatures from the worker and supervisor.
