CPS Due Process - Letter F - Notice of Decision
To Stay
The Form "Letter
F – Notice of Decision to Stay" is generated by TFACTS application
and will be sent to the to the Substantiated Perpetrator who is the focus
of the due process formal file review.
Purpose of this form is to send notification to the Substantiated Perpetrator
who is the focus of the due process formal file review that due to certain
events (arrest or involvement in legal proceedings) listed on the letter,
the formal file review process has been 'stayed' (put on hold) until the
events have been resolved. This form should be generated at the same time
as CPS Letter 2 and should automatically save a copy of the letter in
TFACTS when generated.
Datethe system
defaults the current date into the field and allows the user to change
to a future date but the user cannot enter a post date. Required
field.
Certification Numberif the letter
is sent to the recipient via Certified Mail, enter the mail receipt number
here. The 'Certification Number' entered by user will be displayed on
the generated form. User can enter up to a 30 character alphanumeric certified
mail number. This
will be displayed to the right of the 'Certification Number' label. This
is an optional field. If user does not enter any value in this field at
time of generation of this form then field does not show the label on
the generated letter.
The
Department has been notified thatthis section contains two check boxes with
a description of the reason why Due Process is being stayed. The user
must select one or more of these check boxes in order to generate the
letter. Boxes checked will display, along with the related description,
on the generated form.
Phone Numberenter the work telephone number for the employee
who is designated as the 'Reviewer' on the due process record.
Perpetrator Namename of the Substantiated Perpetrator who
is the focus of the Due Process event.
