Diligent Search Checklist CS-0584

CS-0584 is used to request information about any person that is associated with the case. The person can be anyone such as Alleged Child Victim, Alleged Perpetrator, or Parent, etc. This form is used by the Family Service Worker (FSW Family Services Worker) in gathering the information related to a case. The information gathered through this checklist is used by FSW Family Services Worker in a thorough search related to the particular case/child and helps in future reviews and decision making for the client(s).

Case tab > Workload tab, click case record ID > Case Overview screen, click Investigations Linked in link menu > Linked Investigations screen, click investigation ID > Investigation Overview screen > Investigation Topics screen, click Forms/Reports link.

Report Help Topic Content Issues

Company Name	Enter the name of the company or individual to which this letter will be mailed. Mandatory.
Company Address1	Enter line 1 of the company or individual address. Mandatory.
Company Address2	Enter line 2 of the company or individual address.
City, State, and Zip Code	Enter the city, state, and zip code of the address. Mandatory.
Salutation	Enter the salutation for the letter recipient. Mandatory.
Your Name	Enter the name of the approval authority of this letter. Mandatory.
