Individual Program Plan CS-0228

The Individual Program Plan (IPP Individual Program Plan) is the tool used by DCS as a treatment plan to document the following items for delinquent youth in its secure facilities and DCS group homes:

• The strength Skills, abilities, talents, resiliencies, and resources that have enabled a family to be successful or to overcome adversity in the past. and needs Needs (formerly referred to as Concerns) are behaviors and risks displayed by the child or family. This should include the reasons for DCS involvement with the family/child. of youth

• The provision of services to build on strengths and address needs Needs (formerly referred to as Concerns) are behaviors and risks displayed by the child or family. This should include the reasons for DCS involvement with the family/child.

• The development of success directed goals and objective

• A continuum of care if needed, after care planning and permanency

The content of an IPP Individual Program Plan is determined within the context of Child and Family Team Meeting. It is initially developed when youth is placed in a YDC or DSC group home and the Supervisor approves it.

CS-0228 is a summary form that can be generated for each youth in a DCS Youth Development Center or DCS operated group homes.
