Notice of Action CS-0800

The purpose of the Notice of Action (NOA) is to comply with the provisions of the Grier Consent Decree where it is mandated that DCS provide written notification of any changes (proposed or actual) to the level of care a child has been receiving. The NOA is the official court-approved documentation that DCS uses to notify the TennCare Consumer Advocates and other interested parties of any changes to a child's level of care.

CS-0800 is generated when a placement recommended in the context of a CFTM meets NOA requirements, or when there is a change in the placement of a child who obtains services from the department and that change in placement level of care meets NOA criteria.

Access

Three methods:

??? > ??? > Child and Family Team Meeting History screen, click the report link. Select ”r;Yes” for the Is A Custody / Placement Change Recommended? option.

??? > ??? > Child and Family Team Meeting History screen, click the print link. Select the child name for which NOA form needs to be generated.

??? > ??? > Placement Records screen, click the report link.
