the person in focus who are absent from the household, OR any persons
who are alleged to be parents of the person in focus.

the value that best represents the status of the parent/relative.
Signed
Putative Father Waiver
If
‘Putative’ is selected for Parent/Relative Status, indicate
whether the person has signed a putative father waiver.
Name
In
order to populate the Name field a person search must be completed
and a person selected from search results OR a new person
added. Click ‘Link Person’ button to initiate person search.
Gender
This
displays the gender associated with the ‘linked’ person.
Absent
from Household
Select
the value (Yes, No, or Unknown) to indicate if the person
is absent from the household.
Reason
for absence from Household
If
‘Yes’ is selected for ‘Absent from Household’, enter a note
describing the reason for the absence.

Represents
the date the period of absence from home began.

End Date
Represents
the date the period of absence from home ended.
Comments
Text field for any
