Education Profile - Psycho/Education Assessment Information

The Education module in TFACTS is for capturing details related to the schools a child/youth has attended, his/her performance in school, and any special education needs.   Education Information can be recorded for any person but is normally only done for children/youth who are receiving services from DCS.

Special Education

The Special Education screen allows for the view and capture of information related to Psycho/Educational Assessments administered to the child/youth as well as any Individualized Education Plans documented for the child/youth.

Steps to add Psycho/Educational Assessment

1. Click Add/Psycho Education action button.
2. Select Assessment Completed Date.  This is the ONLY system-required element on this screen. 

a.  Once the Assessment Completed Date is entered, TFACTS auto-populates the Expiration Date for the assessment for 3 years from the date of assessment.

3. All other fields are optional.

4. Select a value for Completed By to indicate who performed the assessment.  Choices are: Contract Provider and School.
5. If Contract Provider is selected, the Search Provider action button will enable.  Click the button and search for the provider that performed the assessment.  Once the provider is selected from search results, TFACTS will populate the provider name and identifying information in the assessment record.
6. If School is selected, the Search School button will enable.  Click the button and search for the school that performed the assessment. Once the school is selected from search results, TFACTS will populate the school name and identifying information in the assessment record.
7. Click Save.
