Process: Record a Youth Level of Service assessment

2. In the Prior and Current Offenses/Dispositions table, check all items that apply to this youth.

3. Enter a summary description related to the items you selected in step 2.

5. Click Apply.

6. Click the Family Circumstance/Parenting link and complete the required details.

   a.	In the Family Circumstances/Parenting table, check all items that apply to this youth.
   b.	Enter a summary description related to the items you selected in step a.
   c.	Enter sources of information about the family and parenting, if available.
   d.	Click Apply.

7. Click the Education/Employment link and complete the required details.

   a.	In the Education/Employent table, check all items that apply to this youth.
   b.	Enter a summary description related to the items you selected in step a.
   c.	Enter sources of information about the education or employment, if available.
   d.	Click Apply.

8. Click the Peer Relations link and complete the required details.

   a.	In the Peer Relations table, check all items that apply to this youth.
   b.	Enter a summary description related to the items you selected in step a.
   c.	Enter sources of information about the peer relations, if available.
   d.	Click Apply.

9. Click the Substance Abuse link and complete the required details.

   a.	In the Substance Abuse table, check all items that apply to this youth.
   b.	Enter a summary description related to the items you selected in step a.
   c.	Enter sources of information about the substance abuse, if available.
   d.	Click Apply.

10. Click the Leisure/Recreation link and complete the required details.

    a.	In the Leisure/Recreation table, check all items that apply to this youth.
    b.	Enter a summary description related to the items you selected in step a.
    c.	Enter sources of information about the leisure or recreational activities, if available.
    d.	Click Apply.

11. Click the Personality/Behavior link and complete the required details.

    a.	In the Personality/Behavior table, check all items that apply to this youth.
    b.	Enter a summary description related to the items you selected in step a.
    c.	Enter sources of information about the personality or behavior, if available.
    d.	Click Apply.

12. Click the Attitudes/Orientation link and complete the required details.

    a.	In the Attitude/Orientation table, check all items that apply to this youth.
    b.	Enter a summary description related to the items you selected in step a.
    c.	Enter sources of information about the youth's attitude and orientation, if available.
    d.	Click Apply.

13. Click Next Task> below to continue.
