Enter the children or youth involved in an incident

Process: Record a critical or serious incident report

Follow these steps to enter the children or youth involved in an incident.

1. Click the Child/Youth Involved tab.

2. If no children or youth were involved in the incident, check the Not Applicable box.

3. If children or youth were involved, click Add Child/Youth. The Available Children/Youth screen appears.

4. Check the names of those who were involved in the incident

5. Click Save. You are returned to the Child/Youth Involved tab.

6. Click the details link for a child. The Child/Youth Details screen appears.

7. Complete the details for the child's involvement in the incident, such as restraint, confinement, and injury.

8. Click the Runaway/Escape Information link and complete the details on the screen, such as suspected whereabouts and safety factors.

9. Click OK. You are returned to the Child/Youth Involved tab.

10. Repeat steps 6 – 9 for each child on the tab.

11. Click Apply.

12. Click Next Task> below to continue.
