Record immediate harm factors in a safety assessment

Process: Record a safety assessment

The assessment requires you to identify the safety factors that are currently present. The evaluation of safety factors forms the core of the assessment and leads to the safety response.

Follow these steps to record the safety factors.

  1. Click the Immediate Harm Factors tab. This tab contains Section 1 of the assessment.

  2. Review the instructions under Section 1.

  3. For each numbered statement in the grid, do the following:

    1. Click the statement to expand the list of detailed factors.

    1. Select the check box for each factor that applies to the question.

    1. Click select. The Immediate Harm Factor Details screen appears.

    2. Enter a detailed narrative to provide more information about the factors you selected.

    3. Select Yes or No in the Response field, to indicate whether the statement is true for the participants.

    4. Select the Include Narrative for FFA if appropriate.

    5. Click OK. You are returned to the Immediate Harm Factors tab.

  4. Repeat step 3 for each statement.

  5. Click Apply.

  6. Click FFA Narrative. The harm factors you selected with a response of Yes are listed.

  7. Enter a detailed explanation for each harm factor that needs to be copied to the FFA.

  8. Click Apply.

If you select a "Yes" response for any of the immediate harm factors, you must also select one or more interventions on the Safety Interventions tab. See Record safety interventions for a safety assessment.
