Record a transitional survey
A transitional survey is completed on client as he or she transitions out of custody by DCS. The survey captures information that is critical for various reporting purposes. The survey also helps to verify all the needs of the client were addressed when he or she leaves DCS care. A transitional survey can be completed on any child, less than 22 years of age, who receives services from the Department. A transitional survey can be completed on a closed case.
In TFACTS, the transitional survey consolidates the capturing of data for National Youth in Transition Database (NYTD), Transitional Surveys, the Transitional Living Essential Document List, Form CS-0505, and the Post Custody Discharge Summary.
Follow these steps to record a transitional survey for a transitioning child.
From the Home screen, click the Case tab. The Case Workload screen appears.
Open the list of cases by clicking next to your name. The list of your assigned cases appears.
Click the case ID number link for the case you want to view. The Case Overview screen appears.
Click Transitional Survey in the link menu. The Transitional Survey Search screen appears.
Enter the client name and survey type to find existing surveys and click Search. The matching surveys appear in the search results grid.
To record a new survey, select a client name and type of survey and click Add Transitional Survey. The Transitional Survey Detail tab appears.
Click Link Address to select the client's current or forwarding address. The Address Search screen appears. When you select the address you are returned to the survey details screen.
Click Link Person to select the conservator. The Person Search screen appears. When you select the person you are returned to the survey details screen.
Click Link Address to select the address for the conservator. The Address Search screen appears. When you select the address you are returned to the survey details screen.
Click Add Significant Adult to select an adult who was involved with the child. The Significant Adult Details screen appears.
Enter narratives about the permanency goal outcome and education plan.
Under Acquired Job Skills, identify the job skills of the child, if appropriate.
Select the Last Grade Completed by the child.
Click the Transitional Survey Section tab.
Select the survey section you want to complete.
For each question in the grid, select the answer.
Click Additional Information to record more details about the response. The Additional Information screen appears. Enter the narrative and click OK. You are returned to the Transitional Survey Section tab.
Click Apply to save the survey.
Repeat steps 14 – 17 for each section in the survey.
Click Approval to submit the survey to the team leader for approval.
