Putative Father Request Information

On this screen you view or record the details of the putative father search. Multiple putative fathers can be listed on this screen. For each potential father, you enter a cleared or not cleared result and other information.

The Putative Father Worker conducts a search from this screen, enters details of the results, and enters the cleared or not cleared response.

The system performs four types of searches:

Child Name & Gender Match – The system should return all results where the Child’s Last Name, First Name, and Gender matches in both the Request and eligible Registry records.
Birth Place and Birth Date Match – The system should return all results where both of the following two conditions are true. The City and State fields for the Birth Place matches on both the Request and eligible Registry records. The Birth Date on the Request record is no more than 90 days after or 60 days before the Birth Date on the eligible Registry record.
Mother Name Match – The system should return all results where the Mother’s Last Name and First Name matches in both the Request and eligible Registry records.
Father Name Match – The system should return all results where the Father’s Last Name and First Name matches in both the Request and eligible Registry records.

The requestor can view the results of the search and the response information.

When you saves the screen, the status of the request record changes to "Responded." the requestor can view the search results and response details.

If a putative father was identified in the registry search, the putative father can notify DCS that he has fathered the child and will claim responsibility for that child. In TFACTS you record the information from individuals who intend to claim paternity of the child.

Cleared?	Select Yes if the putative father is cleared.
Birth Certificate Exists	Select this check box to indicate the putative father has a birth certificate.
Father's Name on Birth Certificate	Enter the name as it appears on the birth certificate.
Notice of Intent on File	Select this check box to indicate the father has submitted a notice of intent to take custody of the child.
No Name Exists on Either Registry	Select this check box to indicate the father's name is not in either registry that you searched.
Additional Information	Enter more details about the search results and response.
