Transitional Survey Search
This screen displays the transitional surveys recorded for any child or youth in the case. A transitional survey can be completed on any child or youth who receives services from DCS and is less than 22 years of age. Transitional surveys can be completed on a closed case. You can record new surveys, select existing surveys to view or update details and delete surveys that have not been completed.
Click select to view or update a survey.
Click report to generate and print forms for the survey.
Click Add Transitional Survey to record a new survey.
