Transitional Survey Detail

On this screen you record details about the transitional survey for the youth. A transitional survey can be completed on any child or youth who receives services from DCS and is less than 22 years of age. Transitional surveys can be completed on a closed case. The details in the survey are necessary for the following transitional reports:

Details in the survey include conservator's name and address, significant adults, permanency goal outcome, and job skills.

Click Add Significant Adult to select an adult and relationship to the youth or child.

Fields on this screen

What Was The Permanency Goal Outcome?	Describe the details on whether the child or youth accomplished the goals of the permanency plan.
Name of School...	Enter the name of the school the youth will attend after discharge.
Education Plan...	Describe the education plan for the youth after the youth is discharged.
Acquired Job Skills	Select the job skills the youth has learned while in custody.
Last Grade Completed	Select the last grade the youth completed before discharge.
Date Commissioner Review...	Enter the date when the DCS Commissioner reviewed the case for the youth or child.
