Assessments
This screen provides the means
to add an assessment for a child/family and displays a listing of all
assessments that have been created for a child/family. Assessment
details can be added, updated, viewed, summary reports can be generated
and documents can be attached to assessments; based on a worker's
security level.
Assessment
Filter Criteria to
be used to filter the listing of assessments by create date, assessment
type and assessment status. Using
the filter criteria may be helpful when there are more assessment records
to view than will fit on a single screen.
Assessment
List Information regarding in progress
and completed/approved assessments will display in the assessment list.
If
a Family Functioning Assessment (FFA) has been recorded, it will always
display first in the list. Only one FFA can be recorded for a case. The
FFA cannot be deleted from the case.
Assmtcategory of assessment
(FFA, CANS, Life Skills and Safety).
FAST will display only for persons
for whom a FAST was completed in the Family Case before it was moved to
CPS Investigation or Non-Custody module. FAST assessments entered
can be viewed under the person profile assessment history link.
YLS will display only for persons
for whom a YLS was completed in TFACTS before the YLS assessment was discontinued.
Safety Assessments can also be
found under the CPS Investigation and Non-Custody module. The newer
assessments entered can be viewed under the person profile assessment
history link.
Typewill indicate
whether the assessment is the Initial, a Re-assessment, Discharge, Transition
or other type depending on the category of assessment.
Namedisplays the
name of the person for whom the assessment was created. (FFA
displays ‘Family’ since that assessment covers the entire family unit
and not just one person.)
Create Datedisplays the
date the assessment record was created in TFACTS. This does NOT represent
the date the assessment was completed.
Statusdisplays the
current status of the assessment. If the assessment has been
Approved, the date approved will also display in this column.
Resultdisplays overall
assessment results/scores.
The
following actions can be performed from the Assessment List:

or update details (can only be updated if assessment has status of ‘In
Progress’
reportclick to generate
a summary report for an assessment.
documentsclick to initiate
the process for attaching a document to an assessment.
deleteclick to delete
an assessment; which is only visible when the assessment status is ‘In
Progress’.
