Referral Information (FSW)

On this screen you record the details for a placement referral. If the child prefers to be placed in a resource home with his or her siblings, you can enter that preference and select those siblings. Based on the service category and service type, the PSD who continues the placement process can select potential resources to match the child's service needs.

After you complete a referral, the system sends an alert to the PSD, who will take the referral and attempt to complete a placement.

Click Select Siblings to choose one or more siblings with whom the child prefers to be placed.

Fields on this screen

Responsible FSW Family Services Worker	Select the name of the primary worker on this child's case.
Responsible PSD	Select the worker in teh Placement Services Division who is responsible for placing the child.
Placement Needed Date	Enter the date when the child should be placed.
Estimated End Date	Enter the date when the child should be returned to his or her home.
Placement with Siblings Preferred	Select Yes if efforts should be made to place this child with his or her siblings.
Service Category	Select the category of service required for the child.
Service Type	Select the specific type of placement service required.
Additional Comments	Enter any additional details concerning this placement referral.
