Education Passport CS-0657
CS-0657 is a compilation of
documents that constitute the education history of a student or child's
current school record.
The
Education Passport refers to the form and the supporting documents
that are to be provided to a school upon enrollment of a student. All
students in DCS custody as well as those receiving probation services
will have an Education Passport. A
new Education Passport cover sheet is
completed by the Family Service Worker (FSW) whenever a student is enrolling
in a new school. The Education
Passport is placed in the child’s master file. A
copy must be given to foster parents by the FSW.
The parameter screen for the

generating the passport.

Child/Youths Status
Line one: All
students in custody under age 3 must be referred for Tennessee Early Intervention
Services (TEIS). For
these children, the file should contain the completed Education Passport
cover sheet and the TEIS information.
Files
for children from 3 to 5 years of age who are not yet in school will contain
only the Education Passport cover sheet with this box checked. If students ages 3 to
5 are receiving special education services from the public school, the
file should contain a completed Education Passport cover sheet
and the appropriate special education information.
Files
of students of school age (ages 5 and up) should contain a competed Education
Passport cover sheet and all of the necessary documentation.
Files
of students on probation should contain an Education Passport cover
sheet and only current report card/current attendance data.
Line
two:Include in the
file proof of any high school diploma/credential earned by the student. Place
a copy of the high school diploma, the GED/HiSET, or other type of diploma
with the last Education Passport cover sheet/documents and mark the appropriate
box. (Note: if a student enters custody (or another DCS service)
with a diploma, this can be the only document with the Education Passport
Cover sheet unless additional course work is completed in college, technical
school, etc.)
