Parental Status

Select the appropriate description of the parental status. This field is required.

Name

Enter the parent's name. Click Add Person to find a person who exists in the database already.

Gender

Select the parent's gender.

Absent from Household?

Select whether the parent is currently absent from the household.

Reason for Absence from Household

Enter the reason  the parent is absent.

Absence Begin Date

Enter the date when the absence started.

Absence End Date

Enter the date when the absence ended, if applicable.

Comments

Enter additional information about the absent parent.
