Family Functional Assessment CS-0777

The "Family Functional Assessment" is a process and an assessment tool designed to help guide staff in gathering information on the past, present and future of a family by:

• continuously evaluating the family's strengths, underlying needs Needs (formerly referred to as Concerns) are behaviors and risks displayed by the child or family. This should include the reasons for DCS involvement with the family/child., concerns, and challenges,

• always building supportive teams with children and families which will last beyond agency involvement, and

• collaboratively developing individualized plans utilizing family strengths and solutions.

CS-0777 describes the on-going process of organizing and analyzing the family information gathered throughout the life of the case. It can include the previous 12 months of records or the entire historical record.
