Select Yes if the youth waived his or her right to a hearing.

Youth Desire to Wait...

Select Yes if the youth wants to wait at least 24 hours before attending the hearing.

Report Issued Date/Time

Enter the date and time when the report was issued to the youth.

Available Charges

Contains the possible charges for selection. Click one or more items in the list and click Add> to move those items to the Selected Charges  field.

Selected Charges

Contains the charges against the youth.

Reviewed By

Enter the name of the person who reviewed the report.

Title

Enter the title of the person who reviewed the report.

Reviewed Date

Enter the date when the person reviewed the report.
