Record a youth disciplinary report
Facility Name
Select the name of the facility where the violation occurred.
Facility Location
Select the specific site of the facility.
Facility Sublocation
Select the sublocation of the facility.
Charge Date/Time
Enter the date and time when the charges are made against the youth.
