Record a review for a youth's IL eligibility

Follow these steps to record a review for a youth's IL eligibility.

1. From your Home Workload screen, select an Ongoing case.  The Case Overview screen appears.

2. Click IL Program Eligibility and Service Referral in the left menu. The IL Program Eligibility and Service Referrals screen appears. This screen displays the eligibility records for case members.

3. Click program review for an eligibility record you want to review. The IL Program Review History screen appears.

4. Click Add Program Review to record a new appeal or program review. The IL Program Review Decision Details screen appears.

5. Complete these fields based on the type of review and the review form distributed to the reviewers:
   
   Program Review Type   
   Notice of Denial, Termination, or Change Sent On   
   Program Review Request Form Sent On   

6. Enter the decisions and comments from the various reviewers.

7. Click Save. You are returned to the IL Program Eligibility and Service Referrals screen.
