Putative Father Request

On this screen you record or view the details of the search request. Details include the requestor's name, organization, request reasons, and child information.

When you save the details on the screen, the system submits this request to the Putative Father Worker. The PFW will conduct the search and enter a response in the system.

How do I...

Fields on this screen

Request Date	Displays the date when the request was submitted.
Requestor's Last Name	Enter the last name of the requestor.
Requestor's First Name	Enter the first name of the requestor.
Requesting Organization	Enter the name of the organization which submitted the request. Click Link Address to select the address of the organization.
Available Reasons	Contains the possible reasons the organization is making the request. Click one or more items in the list and click Add> to move the reasons to the Selected Reason box.
Selected Reasons	Contains the reasons for the request. To remove a reason, select the item and click <Remove.
Child's Information	Enter the name, gender, birth date, and birth place details for the adoptive child.
Mother's Information	Enter the name details for the child's mother.
Father's Information	Enter the name details for the child's father.
