FAST - Caregiver
On this screen you answer the survey questions related to the caregiver that you selected on the FAST Participant(s) tab. The Definition area defines the meaning for each answer column in the survey. After all parts of the survey are complete, the system calculates an assessment score. If necessary, you can enter additional information about the answers in a comment field. These comments are not included in the scoring calculation.
