Supporting Information Details

The Permanency Plan Supporting Information Details page allows the user to record required plan elements not captured in other sections of the plan or in other modules in TFACTS.

For this screen related to the custody goal participant, the user must select at least one of the "EPSD&T" statements.

Supporting Information page is dynamic and the questions/topics will differ based upon the type of goal(s) associated with the goal participants on the plan.

Fields on this screen

Topic	Details
Plan Date	System displays the date of the Perm Plan.
Status	System displays the status of the Perm Plan.
Child Name	System displays the associated child’s name.
Person ID	System displays the associated child’s TFACTS person identification number.
DOB	System displays the associated child’s date of birth.
Age	System displays the associated child’s age.
Selection Boxes	Select only one of the appropriate selection boxes.
Scheduled Appointment Date	If the EPSD&T selection box associated with this field is selected then the user is required to enter the scheduled appointment date for the associated child’s EPSD&T.
