Record a family permanency plan

The family permanency plan in TFACTS helps the Department to address the overall strengths and needs of the family or case participants. The family permanency plan is typically recorded after the assessment process has been completed. Please refer to policy 31.1 Family Permanency Plans for details about time-frames to record the Family Permanency Plan. See About permanency plans for more details.

Process Overview

Follow these steps to record a new family permanency plan for a case.

  1. From the Home Workload screen, locate and select an Ongoing case. The Case Overview screen appears.

It is helpful if you have entered your Permanency Goals, Strength and Concern Records, and Visitation Plans (if applicable) prior to entering information in the Permanency Plan section.

  1. Click Permanency Plans in the link menu. The Permanency Plans screen appears.

  2. Click Add Permanency Plan to record a new permanency plan. The Permanency Plan Topics screen appears.

    Click the links below or click Next Task> below for more help on entering information in the various sections of the family permanency plan.  

  3. Enter goal participants and plan participants.

  4. Enter supporting information.

  5. Link strengths and concerns.

  6. Link visitation plans.  

  7. Enter signature information.  

  8. Submit the family permanency plan for approval.

  9. (Optional) Enter court ratification details for a family permanency plan.
