Adoption Subsidy Review

This screen allows you to record a review of a subsidy for IV-E Adoption Assistance (AA). You can determine whether a child is still eligible for the subsidy and record modifications to subsidies or AA agreements on this screen. A subsidy review may be completed at any time, but must be completed at least once a year.

Click edit age qualifications to identify the reasons the child can receive the subsidy after age 18.

Fields on this screen

Form/Request Date Received	Enter the date the review form or the date of the Department's or family's request for a modification to the subsidy agreement.
Review Date	Enter the date of the review.
Termination Mailed Date	Enter the date the Department sent a letter to the family indicating that the subsidy agreement was terminated.
State Hearing Requested Date	If the family is requesting a hearing with the State, enter the date the family made this request.
Adoptive Parents/Guardians Still Supporting Child	Select this check box If the family is requesting the subsidy to be continued.
Adoptive Parents / Guardians Still Legally Responsible for Child	Select this check box if the family is still legally responsible for the child.
Action	If the subsidy agreement is being modified, suspended, or terminated, select the action being taken.
Action Reason	If the subsidy agreement is being modified, suspended, or terminated, select the reason for the action.
Action Date	Enter the date the action was taken.
Requested Per Diem Amount	If the agency or family is requesting a change to the amount of the subsidy, enter the amount requested.
Negotiated/Recommended Per Diem Amount	If the agency or family is requesting a change to the amount of the subsidy, enter the new amount of the subsidy.
Approved Per Diem Amount	Enter the approved per diem amount.
Approved/Denied Date	Enter the date of the approval or denial.
