Race

Check the boxes that match the races applicable to the participant.

Hispanic/Latino

Select Yes or No to indicate whether the participant is of Hispanic or Latino origin.

Available Ancestry

Contains the possible ancestry types for the participant. Select an option in this box, then click Add> to move the ancestry type to the Selected Ancestry box.

Selected Ancestry

Contains the ancestry types that have been designated for the participant. To remove a type, select it, then click <Remove.

Other Ancestry

If the provided ancestry types match the participant, enter the ancestry.

Tribal Affiliation 1, 2

Select up to two tribal affiliations for the participant.

Available Languages

Contains the possible languages the participant speaks. Select a language in this box, then click Add> to move the langauge  to the Selected Language box.

Selected languages

Contains the languages that have been designated for the participant. To remove a language, select it, then click <Remove.

Primary Language

Select the primary language spoken by the participant.

Other Language

Enter the language of the possible language selections do not match the participant's language.

Interpreter Needed

Check this box if the participant needs Needs (formerly referred to as Concerns) are behaviors and risks displayed by the child or family. This should include the reasons for DCS involvement with the family/child. an interpreter.

Interpreter Type

Enter the type of interpreter.

Religion

Select the religion of the participant.

Other religion

Enter the religion if the Religion field does not contain the correct religion for the participant.

Gang Member

Indicate whether the participant is a gang member.

If yes, what gang affiliation

Enter the name of the gang.

Disaster Victim

Select the name of the disaster if the participant is a disaster victim.

Deceased

Check this box if the participant is deceased.

Deceased Date

Select the date when the participant died.

Cause of death

Enter the cause of participant's death.
