Alert Type

Select the specific type of alert, such as Seizures.

Start Date

Enter the date when this alert should start appearing in the youth record.

End Date

Enter the date when this alert should be removed from the youth record.

Reason for Alert

Describe why the alert is needed or describe steps that staff should take for ensure safety for the youth and others.

Reason for Ending Alert

If you are ending the alert but not deleting the record, describe why the alert is being canceled.
