Race

Check the boxes next to the person's race. You can check more than one box.

Hispanic/Latino

Select Yes or No to indicate whether the person identifies himself or herself as Hispanic or Latino.

Available Ethnicity

Displays the possible ethnic origins in the system. Select each ethnic origin that applies to this person, then click Add>. The ethnic origins you selected moves into the Selected Ethnicity list.

Selected Ethnicity

Displays the ethnic origins selected for this person. To remove an ethnicity, select it, then click <Remove.

Other Ethnicity

Enter the ethnic origin if the Available Ethnicity list does not contain the correct ethnic origin.

Tribal Affiliation 1

Select the name of the primary tribe in which the person claims membership. These are ICWA values.

The system alerts the supervisor and worker when Indian tribe information is entered.

Tribal Affiliation 2

Select the name of a tribe if the person claims membership in another, secondary tribe. These are ICWA values.

Available Languages

Displays the possible native languages in the system. Select each language that applies to this person, then click Add>. The languages you selected moves into the Selected Ethnicity list.

Selected Languages

Displays the languages selected for this person. To remove a language, select it, then click <Remove.

Primary Language

Select the primary language the person speaks.

Other Language

Enter the language if the Available Languages list does not contain the correct language.

Interpreter Needed

Check this box if the person needs Needs (formerly referred to as Concerns) are behaviors and risks displayed by the child or family. This should include the reasons for DCS involvement with the family/child. an interpreter when interacting with agency personnel.

Interpreter Type

Select the type of interpreter.

Religion

Select the person's religious affiliation.

Other Religion

If you selected Other in the previous field, enter the name of the person's religion.

Gang Member

Select whether the person is in a gang.

If yes, what gang affiliation?

Enter the name of the gang if you indicate the person is a gang member.

Disaster Victim

Select the name of the disaster the person was involved in, if applicable.

Deceased

Select this check box if the person has died.

Deceased Date

If the person is deceased, enter the date on which he or she died.

Cause of Death

Enter the cause of the person's death.
