On this screen you enter details for a person's care coordination note. Once the note record is saved, the record cannot be updated. To modify information, you enter an addendum to the note.

Click Add Addendum to enter a change to a saved care coordination note.

Category	Select the category for this health note.
Type	Select the type of note.
Health Specialist	Select the type of specialist for the note.
Date	Enter the date you entered this note.
Time	Enter the time you entered this note.
Note	Enter the contents of the note.
Service Need Identified	Check this box if a service need is identified with this health note.
