Home Study Type

Select the type of home study you are recording. If you are adding an initial home study, the system displays "Initial."

Assessor

Select the name of the worker who conducted the home study. The field defaults to the name of the  worker who is currently logged on.

Begin Date

Approval Types

Contains the checkboxes for possible approval type for the home study.  Check each box that applies.

Addendum Information

Contains the checkboxes for the reason to record an addendum. If amending a home study record, you must select at least one reason.
