Record health services

Process: Record a health profile

Follow these steps to record health services received by a child or youth in custody.

  1. On the Health Profile screen, click the Health Services tab. The Health Service History screen appears. This screen contains a list of completed health services and pending health services. You can view a summary of all services and add new services.

  2. Click Add Service to add a new health service for the person. The Health Service Detail screen appears. This screen contains several areas for recording service details, including service details, EPSTD, good cause exceptions, and adverse actions.

  3. On the Health Service tab, complete the details for a health service record.

    1. Complete the required fields:

      Category
      Type
      Begin Date
      Provider Name
      – Click Search Provider to select a provider in the system or add a new provider for this service.

    2. If you have the information for a completed service, record additional details.

    3. Click Apply.

  4. Click the EPSTD tab. In the EPSDT Information screen complete the details and click Apply.

  5. Click the Good Cause Exceptions tab. In the Good Cause Exception screen and complete the details and click Apply.

  6. If you need to record appeal information for the service, click the Adverse Actions tab. In the Adverse Actions screen  complete the details and click Apply.

  7. Click Next Task> below to continue.
