Intake - Basic Information

This screen allows you to record the basic information collected during an intake. Intakes that come into the agency are routed to a screener, who collects the information and records it in TFACTS. Intakes may be in the form of telephone calls, letters, face-to-face visits, faxes, email, or any other electronic method. See About intakes.

This screen is also where you indicate that the intake is complete and ready for the screening decision maker to make a decision regarding this intake.

Fields on this screen

Intake Date/Time	The system automatically enters the date and time when the intake was recorded. To change it, enter the date and time when the agency took the referral. You cannot enter a future date.
Intake Method	Select the method by which the agency received the intake. This field is required.
Intake Category	Select the general category of the intake. The selection made will drive what intake screens and fields are enabled. If you leave this field blank, all of the intake screens and fields are enabled.
Intake Narrative	Enter the information the referent provided concerning this intake. While you can save the intake record without entering a narrative, you must enter the narrative in order to complete the intake.
Safety Hazards	Enter a description of the safety hazards identified by the referent.
Living Arrangement at Time of Intake	Select the value that best describes the living arrangement in the home at the time the intake was received.
Submit for Approval	Select this check box when you have finished entering all of the intake details. When this box is checked and the intake is saved, it will appear on the intake workload with a status of completed. This status indicates that the intake is ready for a screening decision maker to review and make a decision.
