Prescribed By

Click Search Provider to select the name of the physician who prescribed this medication.  

Quantity/Refills

Enter the number of refills that were prescribed.

Begin Date

Enter the date when the person started taking this medication.

End Date

If the person no longer takes this medication, enter the date the person stopped taking it.

Special Instructions

Enter a reason for taking medication, any reactions to the medication, any misuse or inconsistency in the use of the medication, and so on.

Approved Date

If the medication is psychotropic, enter the date when the medication was approved.

Comments

Enter additional information about the psychotropic medication.
