Primary

Select this check box if this is the person's primary address. Only one address may be identified on the person profile as the primary address.

Address Type

Select the type of address.

C/O

If mail is sent in care of another individual, enter that individual's first and last name. You can enter up to 250 characters.

Effective Date

Enter the date this address was in effect. Use the format MM/DD/YYYY.

End Date

Enter the date this address was no longer in effect. Use the format MM/DD/YYYY.

Delivery Detail

Enter any additional information or delivery instructions. You can enter up to 250 characters.
